What is this fault likely to be?

Gear changes are smooth (better when car warm) but car does not like to idle in drive/gear. Idles better when put into neutral.

Is this an engine idling fault or internal gearbox fault?

Engine idle fault.Give the box and engine a good service...Is there a step up solenoid on these?????

Mine does the same thing. I just put it in neutral.

Check all your vacuum hose to make sure theres no leaks and the tube is in good condition, also your gearbox oil level
