As I understand it, that's the whole problem in the third world. No one is bold enough to tell them how to use them properly.
I have heard stories of aid organisations showing indian women how to put them on their husbands by demonstrating on a banana. 12 months later the people came back and all the women had had another baby. When they asked them about the condoms the indian women told them they had it put on a banana just like they were shown and left the banana next to the bed during sex.
